Cardiac Education Program

Our cardiac education program assists patients with cardiac disease to learn to manage their disease and to help slow down progression of their disease. Our nurses educate patients about behavior modification concerning diet, activity and medication compliance. They also use their assessment skills in monitoring the cardiac status of their patients. Significant changes in the condition of the patient are communicated to the physician, to prompt efficient treatment that will prevent re- hospitalization if possible.

Diabetes Education Program

Our diabetes education program assists patient with diabetes and their families in learning to manage their diabetes. Our certified diabetes educator is available to our nurses and patients to assist with educating patients about diet, exercise, self- blood glucose monitoring, and about complications of uncontrolled diabetes. Patient teaching materials are current and offer information on all aspects of diabetes management including: foot care, eye care, sick day management, and acute complications.

Smoking Cessation

Our smoking cessation program provides homebound patients with chronic disease, such as COPD, with assistance in tobacco cessation. Our tobacco cessation specialist provides nicotine replacement therapy at no cost to the patient through EAHEC ( Everglades Area Health Education Center) and collaborates with the physician in providing treatment that will assist the patient to quit smoking. Patients are also provided with access to the Florida Quit line for additional counseling.
